Horsham Pathfinder Service

Community Support

Who we are

Richmond Fellowship’s Horsham Outreach Community Based Support Service works with individuals affected by mental ill health already living in their own homes. After your initial discussion, you will be allocated a Recovery Worker who will offer up to 12 support sessions either in person or virtually, working towards individually developed practical recovery goals.

This Richmond Fellowship service is a part of Pathfinder West Sussex, supporting people with their mental health and wellbeing. Pathfinder is an alliance of organisations providing a single point of access to mental health support across West Sussex.

What we do

In our Outreach Support Service, we can help individuals with practical goals such as linking in with specialist advisors for up to twelve sessions of one-to-one mental health support including: anxiety, depression, self-esteem, sleep hygiene, coping strategies and stress management.

We can signpost you to specialist services so you can access community, social and leisure activities to tackle loneliness and social isolation, improve your physical health plus short-term problem solving support around a range of issues. We are also able to signpost you for help with support getting employment, money management and housing.

We accept self-referrals, professional referrals and friends/family referrals. To refer, please call 01403 241866 or email Info.Horsham@RichmondFellowship.org.uk
