Horsham Outreach

Community Support

Who we are

Richmond Fellowship’s Horsham Outreach Community Based Support Service works with individuals affected by mental ill health already living in their own homes. After your initial discussion, you will be allocated a Recovery Worker who will offer up to 12 support sessions either in person or virtually, working towards individually developed practical recovery goals.

This Richmond Fellowship service is a part of Pathfinder West Sussex, supporting people with their mental health and wellbeing. Pathfinder is an alliance of organisations providing a single point of access to mental health support across West Sussex.

What we do

In our Outreach Support Service, we can help individuals with practical goals such as linking in with specialist advisors for up to twelve sessions of one-to-one mental health support including: anxiety, depression, self-esteem, sleep hygiene, coping strategies and stress management.

We can signpost you to specialist services so you can access community, social and leisure activities to tackle loneliness and social isolation, improve your physical health plus short-term problem solving support around a range of issues. We are also able to signpost you for help with support getting employment, money management and housing.

In our Peer Support Service, all our staff have lived experience of mental ill health and have used mental health services. Our Peer Recovery Workers share their own experiences in a recovery-oriented way. This is a successful way to enable and support people to explore strategies around improving overall wellbeing, managing symptoms, building confidence and often developing an individual Wellness Recovery Action Plan.

The Emotional Wellbeing Service consists of Mental Health Support Coordinators who provide 1:1 mental health support to patients who are registered with a GP at either Park Surgery, Orchard Surgery, Holbrook Surgery or Riverside Medical Practice in Horsham.

The Mental Health Support Coordinators work with patients to create personalised support plans and deliver person-centred support using a range of coping strategies, grounding and breaking techniques, managing anxiety and depression, self-esteem building, goal setting and signposting. The Mental Health Support Coordinators offer 6 one-to-one sessions working on 1 or 2 personalised mental health goals. The service works in partnership with Sussex Partnership Foundation Trust who provide Mental Health Practitioners who support the Mental Health Support Coordinators with patients who may present with higher support needs.

If you would like to access the Emotional Wellbeing Service, you must be a patient who is registered at one of the 4 GP surgeries listed above. We only accept referrals from surgery staff.
