We provide outreach support to people with mental ill health living in the community.
This Richmond Fellowship service is a part of Pathfinder West Sussex, supporting people with their mental health and wellbeing. Pathfinder is an alliance of organisations providing a single point of access to mental health support across West Sussex. To find out more, visit www.pathfinderwestsussex.org.uk.
How we help
Our Outreach service includes individual sessions with a recovery worker who will work alongside you to address your practical needs and achieve your personal goals. Our service will also support you in respect of your emotional, mental and physical wellbeing, working with and supporting you to access other services to improve over-all wellbeing.
Individuals can access the service by phone, email or attend a library information and advice session at Horsham or Chichester library (see www.pathfinderwestsussex.org.uk for most up to date times).
Who we can help
Our service is for adults aged over 18 with mental health support problems who are registered with a GP and/or are resident in the areas served by Horsham district council.
At the heart of everything we do lies:
- Hope: the belief that recovery is possible for every individual
- Respect: valuing differences and all contributions
- Enabling: giving people encouragement and support to achieve their goals
- Inclusion: the belief that everyone has a right to participate fully in society.
