Milton Keynes Supported Housing
80 Boycott Avenue
At Oldbrook we provide supported housing for adults who need support with daily living and social skills as the result of long term mental health problems.
How we help
At Oldbrook we offer 12 residential places in three terraced houses. Each resident has a lockable room and will share bathroom, toilet and communal kitchen and lounge. There is a large garden at the rear, which the houses share.
Our scheme helps individuals to develop a support plan tailored to their needs. A key worker is allocated to each individual and they work with them throughout their placement on the short and long-term objectives highlighted in this plan. Regular reviews take place to map the progress of each person. We also link in with support networks and agree a weekly service structure that fits with individual needs.
Who we can help
All referrals to our services need to include an application form completed by the service user or their care co-ordinator. Referral packs detailing the required information are available on request.
Referrals to Oldbrook should be submitted via the NHS bed manager: 01908 243 858.
At the heart of everything we do lies:
- Hope: the belief that recovery is possible for every individual
- Respect: valuing differences and all contributions
- Enabling: giving people encouragement and support to achieve their goals
- Inclusion: the belief that everyone has a right to participate fully in society.
